How the QR code works

Anyone who scans the QR code on your card is taken directly to your public verification page. The page shows your dog's name, breed, color, your city and state, and the registration number — without revealing your email or payment information.

Summary of general access principles under the Americans with Disabilities Act (ADA). This card is a private identification record.

Service animals are generally permitted to accompany people with disabilities in areas where members of the public are allowed to go.
When it is not obvious what service an animal provides, staff may ask only the limited questions permitted under the ADA.
Under the ADA, staff cannot require documentation — such as proof that the animal has been certified, trained or licensed — as a condition of entry.
A service animal can be asked to leave in circumstances permitted under the ADA, including when it is out of control or not housebroken.
