How to Register a Service Dog
A simple, five-step guide to creating a private service dog registration and ID card record in the United States.
Confirm your dog meets your needs
A service dog is individually trained to perform tasks for a person with a disability. Make sure your dog is trained for the work you need before creating an identification record.
Gather handler and dog information
You will need your first and last name, city and state, email address, and your dog's name, breed, color, sex and date of birth.
Upload a clear photo
Take a well-lit photo of your dog and crop it to fit the ID card. The photo will appear on the digital card, printable card and certificate.
Review and pay the one-time fee
The registration fee is $19.99 USD, paid once per dog. There is no subscription or recurring charge.
Receive your documents by email
Within minutes you will receive your digital ID card, printable front/back card, registration certificate, QR verification link and Wallet passes.
Important reminder
Registration is voluntary and not required by law. Service Dog ID does not certify or evaluate your dog. The ID card is a convenience tool, not proof of legal status.
